چکیده

case presentation an 85 years old patient presented because of intractable right leg pain with minimal activities. she was diagnosed with significant right foraminal stenosis and she failed conservative non-invasive therapies. conclusions percutaneous lumbar hydro decompression can be a promising method for the treatment of the patients with lumbar spinal stenosis and neurogenic claudication secondary to ligamentum flavum hypertrophy. introduction lumbar spinal stenosis with symptomatic neurogenic claudication is a frequently encountered clinical entity which requires endoscopic or open spine surgery once the patients fail to respond to conservative therapies. in this case report we present a novel minimally invasive decompression technique using a fluid jet device (hydrocision inc, billerica, ma, usa) for a patient with symptomatic spinal stenosis secondary to ligamentum flavum hypertrophy. to our knowledge this approach has never been described in the medical literature.

Hypertrophy of ligamentum flavum in lumbar spinal stenosis associated with increased proteinase inhibitor concentration.

BACKGROUND It is well known that age-related fibrosis, or decreases in the elastin-to-collagen ratio of the ligamentum flavum, along with hypertrophy of the ligamentum flavum, are associated with lumbar spinal stenosis. However, the molecular mechanism by which this fibrosis and hypertrophy develop is unknown. Tissue inhibitors of matrix metalloproteinase (TIMPs) are proteinase inhibitors that ...

Percutaneous disc decompression for the treatment of lumbar spinal stenosis.

Percutaneous disc decompression was used to treat three patients with severe spinal stenosis using Nucleoplasty. Access to the disc was obtained bilaterally using a paramedian approach. Channels were created bilaterally using Coblation and stabilized using coagulation. The patients were reassessed at 1 week, 2 weeks, 4 weeks, 6 weeks, 6 months, and 1 year. MRI was obtained on the first two pati...

MicroRNA-221 Regulates Hypertrophy of Ligamentum Flavum in Lumbar Spinal Stenosis by Targeting TIMP-2.

STUDY DESIGN A study of lumbar ligamentum flavum (LF). OBJECTIVE The aim of this study was to identify LF hypertrophy related microRNAs (miRNAs) expression profile and to investigate the role of miRNAs in the development of LF hypertrophy in lumbar spinal stenosis (LSS). SUMMARY OF BACKGROUND DATA Although histologic and biologic literature on LF hypertrophy is available, the pathomechanism...

The paradoxical relationship between ligamentum flavum hypertrophy and developmental lumbar spinal stenosis

BACKGROUND Ligamentum flavum (LF) hypertrophy is a common cause of lumbar spinal stenosis and is thought to be degeneration-driven. Developmental spinal stenosis (DSS) is characterized by pre-existing narrowed spinal canals and is likely a developmental problem that occurs in childhood. In these cases, the LF may demonstrate different characteristics as compared to degeneration-driven stenosis....

mild(®) Lumbar Decompression for the Treatment of Lumbar Spinal Stenosis.

More than 1.2 million people are undergoing treatment for lumbar spinal stenosis (LSS) in the United States. Yet, therapeutic options for these patients are limited to either conservative treatments or highly invasive surgeries. A new image-guided interlaminar decompression procedure, mild(®), offers significant relief for many of these patients by debulking dorsal element hypertrophy while pre...
